Washington DC Birth Certificates

To obtain a Washington DC birth certificate, you need to visit the Vital Records Division of the State Center for Health Statistics. They can help you obtain or revise birth and death records.

First of all, you need to fill in an on-line form for Birth Certificate Request from State Center for Health Statistics Administration. This application form is for DIstrict of Columbia Occurrences only

In the form, you need to fill in the personal information about the certificate holder, such as name, DOB, Sex, Hospital, City, Father's Name and Mother's Maiden Name.

For each of the origical Washington DC birth certificate requested, USD$23.00 is required and for Computer Certificate Short Form, it will be USD$18.00 each.

After you have fill in the form and sign it, you can mail it with your photo ID and payment to:
